Eldercare Services builds on our highly acclaimed Professional Care Management and Homecare services that we have been providing for 23 years by now offering a third option of care: Willow Bridges Concierge.
Each care plan is individually tailored to provide assistance to your family and loved one in three ways:
• Homecare
• Care Management with or without Homecare
• Willow Bridges Concierge
Depending upon your family and your loved one’s wants/needs, any of the three options may be the best solution. Willow Bridges Concierge provides the highest quality of care along with fulfilling the wishes and dreams of each of our members. Professional Care Management with or without Homecare and stand alone Homecare continue to be the “Best in Class” solutions for the types of needs that they serve.
Willow Bridges Concierge
Willow Bridges is a high-touch, highly personalized designed service for adults of all ages who wish to stay in their homes with the highest quality of life possible, and all the days of their lives. Willow Bridges Concierge members are adults without a family advocate, who have family at a distance, or adults who just have specific medical or personal lifestyle wants or needs.
Willow Bridges Concierge will be the consultation and resource to solve the smallest issues (such as finding a good cook or planning a trip to the symphony) to the most critical issues (like advocating when our Willow Bridges client is hospitalized). Willow Bridges Concierge ensures the values, dreams and wishes of our Concierge clients are met every step of the way.
For those individuals who want to live without compromises, who desire fulfillment and not just care, Willow Bridges Concierge is the right choice.
Professional Care Management & In-Home care
For families who live out of the area or have busy lives and for circumstances that require in-home assistance, professional guidance and family coaching, we provide a combination of services designed to:
• Work with the family in making informed choices
• Navigate the legal, medical and financial landscape
• AND assist with the challenges of daily living at home
This combination of Homecare and Care Management is designed to work together in support of the family AND the elder.
Professional Care Management
Often a family faces confusing choices for getting Mom or Dad the care they need. In other cases, the biggest challenge is communicating with a loved one who is suffering from a Dementia, such as Alzheimer’s.
In these cases, consulting with one of our certified professionals (licensed Professional Care Managers) will work with a family to reach the best decision about living at home, find the best care facility, or gain tools for communicating in a way that is constructive or may not feel confident in making.
In-Home care
When the primary issue is the challenge of daily living at home – concerns about living safely, assistance with errands, daily chores, companionship, and more – our In-Home Care service may be all that is needed. Knowing that our team of professional care managers can be called upon is a plus for our clients opting for our In-Home Care.
You may also qualify for a Comprehensive Professional In-Home Assessment performed by a certified, Professional Care Manager. Call (866) 760-1808 or Email Us to learn more.
