Moving clients with dementia can be challenging. Alzheimer’s disease and the related disorders often compromise individuals’ reasoning ability which, in turn, creates stress for families who know that a more secure environment is the only safe answer.
To help smooth transitions, Eldercare Services enlists the support of a professional, who interacts with all parties to mediate and remedy any problems that might arise during the first months after the move. With the support of a Care Manager, an experienced Caregiver from our agency can make friendly visits and allow the client to adjust successfully.

This comprehensive service offers a global view of the client’s life with guidance to prepare for the future. The Assessment begins with a personal visit in the client’s living environment to assess how they are managing and discuss the client’s wishes. Information is gathered from those close to the client and from other professionals who may be serving them.
A written summary is provided to address short and long-term care concerns and strategies for handling difficult issues. Recommendations may include home care, relocation, or assisted, supportive living options. Support service referrals may also be made to special agencies or professionals such as attorneys, medical care providers, financial planners, day care options, meal delivery, in home care, or emergency response systems.
The Care Manager allows family members to express and explore their feelings and presents recommendations. This service involves about 10 hours of professional time and helps increase the quality of life and self-esteem of the senior while supporting the family. The Assessment serves as a road map with which the family can self-guide or proceed to navigate with the help of a Care Manager.
We provide advocacy and direct services such as counseling or care giving for adults with developmental disorders, long-standing mental health issues or those under the age of 55 whose care is managed by “special needs trusts.” Professional Care Manager skills are applicable to the needs of adults of all ages. For those with special needs, peace of mind, protecting dignity and self-esteem are the guiding principles.
Our Money Management service provides ongoing assistance with timely bill review, payment and detailed accounting for tax purposes.
An Eldercare Professional will meet with you to guide you as you decide who will make health care decisions in the event that you are unable to decide for yourself. We will help you select that person and prepare the needed documents to assure that your wishes and values are honored. If you request, Eldercare can support your selected representative at the time that their assistance is needed based on your expressed wishes.
Many older adults are entitled to programs that support their financial and care needs. Our professionals consult with the client and/or family members and supply written referrals to appropriate resources.
Many adult children are unable to meet with a Care Manager in person. As an option, we offer consultations, advice and coaching by phone and email. To allow us to give objective feedback, we prefer to meet the elder in person. However, since this is not always possible we have developed “virtual family coaching.” Online coaching is also helpful for adult children who travel frequently or who need periodic guidance.
This program begins with a one-hour consultation with a Professional Geriatric Care Manager to explore future needs and care options based on the older adult’s desires. Annual membership includes quarterly contact and update. Should the need arise; the Care Manager is prepared to step in to advocate for decisions based on your desires and values. Eldercare Club members can attend classes and workshops at no cost.
A Professional Geriatric Care Manager conducts a thorough safety assessment of the older adult’s home and makes recommendations about ways to make the home safer. The Care Manager can also help to implement any of the advised changes.
Eldercare’s Professional Geriatric Care Managers will evaluate and make recommendations for an individual’s necessary level of care based on medical, physical, spiritual and social needs, personal values and financial feasibility. Eldercare Services does not accept referral fees from any source. However, many care facilities (assisted living and small residential homes) extend a 30% discount to our referrals for the first month's rent, representing up to $900 in savings to the client. The discount alone pays for our services. Professional matching reduces the possibility of a disappointing choice and multiple moves.

The Vacation Respite Program provides Care Management and caregiving if needed, in the primary care giver’s absence. Care Managers provide oversight, regular check-ins, a backup plan in case of an emergency, and a caregiver, who can step into a situation temporarily to care for the older adult.
