Care Management is a process of consultation, evaluation, problem-solving and advocacy. Care Managers can provide as little as a single consultation or as much as a lifetime of support. Each client is unique in both needs and desires. Geriatric Care Management encompasses a wide range of support services, which may include:
Care Management provides initial and ongoing evaluation of the client’s situation, which is updated at every visit. Professional Geriatric Care Managers oversee “the big picture” — providing care coordination, monitoring, problem solving, compliance with health-focused routines, appointment calendar management, future needs planning, resource guidance and advocacy.
The Care Manager stays abreast of the elder’s changes and collaborates with the client, the client’s family, and the direct care providers (both professional and personal care providers) to guide care, based on the client’s values and current needs.
Our Care Managers train, supervise, guide and match any needed care givers to each client’s personality, needs and interests. Families are updated and included in the planning to ensure peace of mind and quality of life for the senior. This is especially helpful to families at a distance. Eldercare Services’ Care Managers are available 24/7.
Consultation: The best place to start with a Professional Geriatric Care Manager is with an Eldercare Services Professional Consulation. During the consultation, in our offices or at your home, the Care Manager explores immediate concerns with the family. The Care Manager then helps the family prioritize and develop a plan of action to secure the health and wellbeing of the elder and reduce the family’s stress. Referrals to other professionals such as Elder Law Attorneys or Physicians with geriatric expertise are made at this meeting. Ongoing Care Management will implement the plan of care designed with the family and client to ensure quality of life throughout the journey.
Counseling: Licensed Marriage and Family Therapists and Clinical Social Workers provide support around the often overwhelming feelings associated with “parent care” or “spousal care giving”, such as grief, loss, anger, frustration, depression, disappointment and stress.
