Extending Services, Meeting Wider Needs

Eldercare Services collaborates with physicians, attorneys, CPAs, discharge planners, conservators, facility administrators, HR managers and other professionals who work with seniors and their families.

One of our highly skilled Professional Geriatric Care Managers can act as the liaison between the professional and the senior or senior’s family when necessary. The Care Manager lends expertise to help both parties resolve many senior related issues for the best possible outcome, considering the client’s wishes and recommendations made by the professional.

How it works

  • A physician notices that a patient is missing appointments. The Care Manager arranges help for the client (patient) to arrive at their appointments on time.
  • A discharge planner wants to send a patient home but recognizes that the client may need some help at home. The Care Manager is called in to implement the discharge plan.
  • The HR manager is aware that one of his employees often misses work because the employee is dealing with the needs of an aging family member. The Care Manager is called in to help the employee better manage the situation so that the employee can return to work.
  • Following a recent death, neither the spouse nor the family knows who to turn to for legal or other advice. Dependency, care for the surviving spouse, entitlements and choice of housing are concerns. The Care Manager is brought in to connect and work with attorneys and other professionals as needed.
