Eldercare Services, the proven leader in Geriatric Care Management and Caregiving has an exceptional career opportunity for a Professional Geriatric Care Manager, Geriatric Social Worker or an RN with a focus on Geriatrics.
This person is responsible for directing the care and well-being of our clients who are typically over the age of eighty and have variety of daily challenges in their lives including frailty, dementia and other health related issues. The management required goes well beyond the specific medical issues including all aspects of the person’s life such as social involvement, family issues, etc. He or she provides oversight for all homecare staffing and will collaborate with other Care Managers and Care Supervisors in identifying, evaluating and resolving a variety of care issues. This position will start at 30 hours a week non-exempt and work up to 40 hours a week full-time exempt; this position includes on-call coverage. Full benefits begin at 30 hours a week.

Education and Experience:
- Master’s degree in Social Work, Psychology, Nursing, or related field. Preference to those licensed in their field (PhD, MFT, LCSW, RN, MSW)
- 5+ years direct care and/or care management experience within the past 10 years, including some experience in the geriatric field
- Experience supervising direct caregivers a plus
- An equivalent combination of education and experience will be considered
- Demonstrated clinical knowledge of aging, dementia, and/or disabilities
- Experience working with complicated family systems and family dynamics Essential
Key Responsibilities:
- Comprehensive Assessment of client needs
- Develop and Implement pro-active Care Plans (and caregiving instructions)
- Oversee Personal Attendants (Caregivers) in the execution of caregiving instructions and through regular reporting
- Work within existing family systems to ensure client’s needs are met and families have the tools needed to communicate and work towards realistic goals
