Geriatric Care Managers -through out East Bay
Eldercare Services welcomes applications for Professional Geriatric Care Management. We are leaders in the field of Geriatric Care, offering both Care Management and non-medical attendant care. To be considered, you must be a health and human services professional, with a background in social work, counseling, gerontology, certified care management or home Care Management. Individuals must have at least 3-5 years experience in geriatrics. Experience with family work as well as knowledge of aging resources and options is a plus. This is an opportunity to work one-on-one with our clients and oversee a client centered care plan that focuses on quality of life. Our pay and benefits are competitive. Flexible hours. We are a team spirited, caring group and consider applicants with a like spirit and commitment to elders. We service Alameda, Contra Costa, Marin, San Francisco, San Mateo and Solano Counties.
Minimum Requirements:
- Bachelors Degree in a Human Services Field
- Three to Five years Geriatric Care Management Experience
- Familiarity with Computerized Programs
- Team Player and Dedication to Senior Services

Caregivers
Live-In Caregiver - 24 Hour Assignments
Duties may include assisting the client(s) with daily living activities such as cooking, light housekeeping, running errands, personal care including dressing and bathing, and ambulation assistance. Focus may be on ensuring a safe environment and quality of life for our client. We believe in a client-centered care plan. 24 Hour Live-In assignments are available immediately. Two to 5 days per week.Requires: Compassion, patience, and at least one- year experience as a caregiver. Valid California Drivers License, Reliable Automobile, and Current Auto Insurance required. CNA/HHA Encouraged to apply. Previous experience working with clients with dementia and/or difficult behaviors a plus. Transfer experience including previous experience using hoyer lift desired.
Caregiver-12 Hour Assignments
Duties may include assisting the client(s) with daily living activities such as cooking, light housekeeping, running errands, personal care including dressing and bathing, and ambulation assistance. 12 Hour Day/Night Shift assignments are available immediately. Two to 5 days per week. Our NOC shifts are awake shifts.Requires: Compassion, patience, and at least one-year experience as a caregiver. Valid California Drivers License, Reliable Automobile, and Current Auto Insurance required. CNA/HHA encouraged to apply. Previous experience working with clients with dementia and/or difficult behaviors a plus. Transfer experience including previous experience using hoyer lift desired.
Personal Assistant-Hourly Assignments
Duties may include assisting clients who do not require constant attendant care to engage in daily activities including, errands, medical appointments, social activities, light housekeeping, ambulation assistance, medication reminders, personal care including bathing, and dressing. Hourly assignments are available immediately. Flexible schedules available, consistency required.Requires: Compassion, Patience, Valid California Drivers License, Reliable Automobile, Current Auto Insurance. One year previous caregiving experience. Previous experience working with clients with dementia or difficult behaviors a plus. C.N.A./H.H.A. encouraged to apply.
Friendly Visitor-Hourly Assignments
Duties may include visiting the client and taking him or her on outings. Might read to the client, play games, watch movies, feed the birds, etc.Requires: Compassion, patience, Valid CDL, Reliable Automobile, Current Auto Insurance. One year previous caregiving experience.
We offer and require 6 trainings the first year of employment and ongoing training in following years of employment. Our trainings qualify for C.N.A. CEU's and are Taught by Professional Geriatric Care Managers.
