- Plan of support for primary caregiver
- 30 Days of ongoing support to monitor changes

Plan 3: Whole Life Navigator
90 Day Care Plan Package – 12 Hours Includes:
- Office/Phone Consultation
- Home Visit – Safety Evaluation and Cognitive Screening
- Plan of Care for immediate concerns
- Discussion of entitlements, alternative options for care and local resources
- Plan of support for primary caregiver
- 90 Days of ongoing support to monitor changes
