Traditional Personal Caregiver Plans

If you are looking for Personal Caregiving, we have a variety of options available to suit all ages and needs.   Below are the most common ways in which we can serve you or your loved one.  However, we will work with you to tailor a Home Care plan that best suits your needs, desires and wishes.

In addition, hourly and shift personal caregiving plans may be directed by a Professional Care Manager in collaboration with the client and family. One of our Care Managers can help you design the best plan for your needs.

Plan Options

Typical personal caregiving needs may be:

Hourly Caregiving – Regularly scheduled or as-needed (2 hours or more)

Shift Caregiving:

• 12-Hour Shift – Includes 1 hour of Care Management per month without charge with a full month of service.

• Live In 24-Hours – Includes 2 hours of Care Management per month without charge with a full month of service.

Shift Caregivers must have breaks and sleep periods, but are available to respond when needed.

Specialized Programs

Care Management is available but not required for the following services:

Bathing Assistance
Following a no-cost evaluation by a Professional Geriatric Care Manager, this service provides assistance with all bathing needs.

Home Care Only
Based on specific care needs, caregivers are available without Professional Geriatric Care Management. Ask us for a no-cost evaluation.

Hospice Caregiving
We work with the Hospice team to support clients with qualified caregiving assistance at the end of life.
